Duplicate Licence & Lost Licence
To apply for a Duplicate Licence thats still in date you will need:
- A Dupicate Licence Form D.800 stamped at your local Garda Station(This cannot be downloaded but can be found in Motor Tax Office or Garda Stations).
- You are required to furnish a Passport or a ML10-Certification of Identity by a Member of the Garda Siochana that can verify your identity.
- Fee of 10 euro.
- 2 identical passport photos with a neutral background and both photographs must be signed on the back by the applicant.
- In the case of a change of name the original Marriage Certificate is required.
If your Licence is out of date & lost you require the following:
- FORM D8(A) declaring your licence lost to the Guards
- A learner permit application form or a full driving licence application form which ever is applicable to the application
- You are required to furnish a Passport or a ML10-Certification of Identity by a Member of the Garda Siochana that can verify your identity.
- Fee of €15 for 3 year Full Licence
Fee of €25 for 10 years
Fee of €15 for Provisional Licence - 2 identical passport photos with a neutral background and both photographs must be signed on the back by the applicant.
- If going onto you 3rd or subsequent Provisional Licence, a Fail Cert within 2 years is required or a recent letter/email from RSA saying you have applied for the test and are awaiting a date or have a test date.
