1st Learner Permit

To apply for a 1st Learner Permit you will need:

• Completed Provisional Application Form
• 2 identical passport photos with a neutral background and both photographs must be signed on the back by the applicant
• Eyesight Report-D502  dated within last month
• Theory Test Certificate dated within the previous 2 years
• Original State Birth Certificate or Original Passport 
• Marriage Certificate (if appropriate)
• Medical Report-D501  for classes C, C1, D, D1, EC, EC1, ED and ED1  
• Fee - €15 euro
